NEW PATIENT INFORMATION

When arriving for your initial evaluation at Elite Physical Therapy,  you will be greeted and given new patient paperwork to complete. Please show up at least fifteen minutes early for your scheduled appointment to allow adequate time for completing paperwork and getting any questions you may have answered.

If you wish to fill out paperwork ahead of time, you can print and fill out the documents below and bring them with you on your first visit.

WHAT TO EXPECT ON YOUR FIRST VISIT

During your initial evaluation, your physical therapist will review your paperwork and ask questions about your medical history, symptoms, and health goals. After learning about your history, a comprehensive evaluation will be performed to find the source of your symptoms and any musculoskeletal impairments that may be present. As the evaluation is being performed, your physical therapist will explain all findings and how physical therapy treatment can help with addressing these impairments. During this time you will have the opportunity to ask any questions you may have and learn more about our clinic and the type of care we provide. Future treatment sessions will include hands-on manual therapy, movement education, and therapeutic exercise training to assist with improving your symptoms and meeting your goals.
